SCHOOL PROGRAM AGREEMENT — Let's C.R.E.A.T.E., Inc. (2026-10-school) By activating your account, you acknowledge and agree to the following for your participation in this school-directed program: 1. PROGRAM: Let's C.R.E.A.T.E. is a community-based creative arts and wellness program — NOT a clinical or mental health treatment provider. Your wellness reflections and journal entries are non-clinical, personal creative expression. 2. FERPA / NY ED LAW 2-d: When used by your school, this platform handles your information under the Family Educational Rights and Privacy Act (FERPA) and New York Education Law 2-d / Part 121. Your data is not sold, is not used for targeted advertising or commercial profiling, and is used only to deliver this program. 3. DATA MINIMIZATION: Your school enrolled you with only your name and email. We do not collect race, ethnicity, gender, pronouns, or living situation for school participants. 4. AI DATA PROCESSING: Some features (art coach feedback, journaling prompts) may use AI to respond to content you submit. You may choose to opt out of AI processing of your content. Your choice can be changed at any time in your profile. 5. RETENTION & DELETION: Your data is retained only for the duration of the program. You, your parent/guardian, or your school may request access to, correction of, or deletion of your data at any time. 6. CONDUCT: You agree to uphold the community Code of Conduct and treat others with respect. 7. SUPPORT: If you are in crisis, call 988, text HOME to 741741, or call 911. This program does not replace professional mental health care.
